I acknowledge that I have reviewed and agree to the Refua Terms of Service, including the terms applicable to accessing and using Refua’s platform and healthcare services.

I understand that payment for a Refua service provides access to the applicable service and clinical assessment and does not guarantee a prescription, medication, medical sick note, diagnosis or other particular clinical outcome.

Purpose

Refua provides a technology platform that connects members with independent healthcare practitioners for virtual healthcare consultations.

Understanding virtual care

I understand that virtual consultations may occur by video, telephone or another approved electronic method. Virtual care has limitations, including the inability to perform a full physical examination. The practitioner may recommend in-person assessment, tests, referrals or follow-up.

My responsibilities

I will provide accurate health information, participate from a reasonably private location where possible, and tell the practitioner if I have concerns about the technology or my ability to communicate.

Emergencies

I understand that Refua is not an emergency service. For emergencies I will call 911 or attend the nearest emergency department.

Consent

I consent to receive healthcare through virtual consultations with independent practitioners. I understand I may ask questions and may decline a virtual consultation or request an in-person alternative where available.

Purpose

To coordinate services requested through Refua, relevant personal and personal health information may need to be shared among independent healthcare practitioners and authorized service providers.

Who may receive information

Where necessary for my care or program administration, information may be shared with treating practitioners, pharmacies, laboratories, dietitians, and other providers involved in my selected services, as permitted by applicable law.

Information and safeguards

Only information reasonably necessary for the applicable purpose should be disclosed. Refua’s Privacy Policy explains its information practices, including service providers and possible processing outside Canada.

My choices

I understand that some disclosures may be permitted or required by law without separate consent. Where consent is relied upon, I may withdraw it subject to legal and operational limits; doing so may affect service availability.

Authorization

I authorize the collection, use and disclosure described above to facilitate the services I request, subject to applicable privacy law and the Refua Privacy Policy.

Purpose

This consent relates to assessment for possible Longevity and Concierge weight management program (GLP-1 or related prescription treatment). Treatment is not guaranteed and is prescribed only when an independent licensed practitioner determines it is appropriate.

Benefits and alternatives

The practitioner will discuss expected benefits, realistic outcomes, other treatment options (including lifestyle-based approaches), and the option not to start medication.

Risks and monitoring

I understand that medication may cause side effects, including nausea, vomiting, diarrhea or constipation, and may have more serious risks or contraindications. The practitioner will review my medical history, current medications, pregnancy-related considerations where applicable, warning symptoms and the need for follow-up and monitoring.

My responsibilities

I will disclose my health history and medications accurately, follow prescribing and monitoring instructions, and promptly report concerning symptoms. I will not change my dose without clinical direction.

Clinical consent

I understand that this form does not replace the prescribing practitioner’s individualized informed-consent discussion. I consent to the proposed treatment only after that discussion, with an opportunity to ask questions.

Membership selection

I acknowledge that Refua memberships may be offered for fixed terms of 3, 6 or 12 months. The selected term, total price, applicable taxes and included platform features will be shown at checkout.

Payment and renewal

I authorize the disclosed membership payment using my selected payment method. Unless I cancel renewal before the next term begins, my membership will automatically renew for the same fixed term at the disclosed renewal price. Approved third-party financing is subject to the financier’s separate terms.

Cancellation

To request cancellation, please contact us through Contact Us in My account at least 14 days before renewal. The Refua team processes cancellation; submitting a request does not itself cancel the subscription. Cancellation takes effect at the end of the current fixed term and does not ordinarily refund the current term, except as stated in the Terms of Use or required by law.

Price changes and services

Refua will disclose renewal price changes before they take effect. Membership access does not guarantee a prescription, treatment outcome or practitioner availability. Separate practitioner or third-party fees may apply unless included at checkout.
